DMH PrimeTime located in the Kenwood Medical Center is a free program that provides many benefits for people age 55 and older.

• Professional assistance with Medicare and secondary insurance billing and reimbursement procedures
• Seminars specific to aging
• Help with Living Will and Health Care Power of Attorney
• Free community transportation vans based on availability

Applying For Membership

To take advantage of the benefits offered with a PrimeTime membership, print the form out and send it to DMH PrimeTime, 2300 N. Edward St., Decatur, Illinois, 62526. Please allow two weeks for receipt of your membership card.
